(V.D.04) Healthcare

It is a Home Care Service (SAD), created by the Federal Government with the aim of expanding home care and is available in the Unified Health System (SUS). The program’s slogan “Hospital safety in the comfort of your home” is closely associated with the program’s proposal, as Home Care can be understood as a set of health actions carried out at home, with the intention of reducing the risk of infections, avoiding unnecessary hospitalizations, in addition to improving the management of hospital beds and the use of resources.

The main objective is to promote safe dehospitalization, both in the pre-hospital and post-hospital phases, as early as possible, and to provide multidisciplinary care at home for people who cannot travel to the Basic Health Unit. With this, it helps families and, especially, women who are generally responsible for domestic and care tasks. Access to the SAD is generally done at the hospital where the user is hospitalized or at the request of the Family Health/Primary Care team or the Emergency Care Unit (UPA)

Aim of the Good Practice

The main objective is to promote safe dehospitalization , both in the pre-hospital and post-hospital phases, as early as possible, and to provide multidisciplinary care at home for people who cannot travel to the Basic Health Unit.

Target Group of the Good Practice

It is indicated for people who, being in clinical stability, need health care in a situation of temporary or permanent restriction to the bed or home or in a degree of vulnerability in which home care is considered the most opportune offer for treatment, palliation , rehabilitation and prevention of injuries, with a view to expanding the autonomy of the user, family and caregiver.

Evaluation of the Good Practice

It is carried out by the responsible Technical Area, but the office monitors the implementation of new actions through the SUS management instruments, such as the Annual Management Report.
